Team Registration:

This form must be submitted no later than November 25 and be sure to also submit the "Team Roster" form! At checkin each player is required to sign next to their name before playing your first game. This is to acknowledge they accept the terms & conditions of the waiver at the bottom of this paige.

Once you have submitted this form you will be asked to make payment. When confirmation of payment is received, your spot is reserved! After November 25 all Individual Registrations will be placed on teams that need more players and given team captain contact info.
